What Paperwork Will a Minor Patient Need for Their Appointment?

We will need a copy of the minor's birth certificate, or legal guardian paperwork; a copy of the parent or guardian's Montana ID or Driver's License, or Tribal ID and medical records related to the minors’ qualifying condition.
